Position: SOCIAL WORKER
Responsibilities: We are seeking an experienced social worker to provide comprehensive services for patients and their families in the solid organ transplant program. The social worker must be able to identify and provide ongoing patient care assessment through age-specific intervention, perform comprehensive psychosocial assessments that involve alcohol, crisis intervention, and acute care, counseling, referrals and guidance during hospitalization and outpatient treatment.
Applicant Requirements: Must have a four-year degree in social work or related field, with a Master's degree in social work required. Candidate should have 2-3 years' experience in a hospital or medical setting, and possess a CISW in the State of Arizona.
